The 2027 Claims Linkage Deadline: Why Medicare Advantage Plans Must Act Now or Risk Losing Risk Adjustment Revenue

A policy change buried in CMS's April 6, 2026, Calendar Year 2027 Medicare Advantage Rate Announcement carries one of the most consequential revenue implications for MA plans in recent memory - and most organizations have not yet begun to respond to it.
Beginning in Calendar Year 2027, CMS is finalizing the exclusion of diagnoses derived from unlinked chart review records (CRRs) from risk score calculations. In plain language: if your plan submits supplemental data - chart review findings - without linking those records to corresponding encounter data or fee-for-service claims, CMS will no longer count those diagnoses toward your members' risk scores.
For plans that have built their supplemental data programs around standalone chart review submissions, the financial impact of inaction could be material. The time to redesign those workflows is not next year. It is now.
What CMS Actually Said - and Why It Matters
The CY 2027 Rate Announcement, published by CMS on April 6, 2026, states:
"CMS is finalizing the proposal to exclude diagnoses from unlinked chart review records (CRRs), with an exception for beneficiaries who switch from one MA organization to another."
This applies to both CMS-HCC and RxHCC risk adjustment models for non-PACE organizations. The only exception is for members who transition between MA organizations - for example, a beneficiary who was enrolled in one MA plan in CY 2026 and switches to another in CY 2027. For all other members, unlinked CRRs will be excluded from risk score calculations effective CY 2027.
This policy builds on a direction CMS has been signaling for several years. Since CY 2022, CMS has calculated all risk scores using only risk adjustment-eligible diagnoses from encounter data and FFS claims. The 2027 finalization closes the remaining gap - eliminating the ability of plans to rely on standalone chart review submissions to capture diagnoses that are not otherwise substantiated by linked encounter activity.
Revenue Exposure for Plans That Are Not Linked
To understand the financial stakes, it helps to understand how many MA plans currently rely on unlinked supplemental data submissions.
For years, chart review programs have been a legitimate and widely used mechanism for capturing diagnoses that may not have been coded accurately - or at all - in claims and encounter data. Many plans have operated these programs without systematically linking chart review findings back to specific claims or encounters. The diagnosis capture was real. The clinical documentation supported it. But the linkage to encounter data was absent or incomplete.
Under CY 2027 rules, those unlinked submissions will not count.
Plans that have been capturing meaningful HCC risk through unlinked CRRs - without building encounter data linkage into their workflows - face a direct risk score reduction beginning with the 2027 payment year. Depending on the volume of unlinked submissions and the HCC weights involved, the revenue impact for some plans could run into the millions of dollars annually.
This is not a hypothetical. It is a policy that has been finalized and published. CY 2027 begins January 1, 2027. The data submission window is already narrowing.
Insider Tip No. 1: Audit Your Current Supplemental Data Submission Workflows Now
The first step is an honest assessment of your current state. Pull a representative sample of your most recent chart review submissions and answer the following questions:
What percentage of submitted diagnoses are linked to a corresponding encounter record or FFS claim?
What percentage are submitted as standalone chart review records with no encounter linkage?
What is the HCC weight and estimated payment value of diagnoses that currently rely on unlinked CRRs?
This audit will tell you precisely how exposed your plan is and where the highest-value remediation opportunities lie. Plans that conduct this assessment in the summer and fall of 2026 will have time to build solutions. Plans that conduct it beyond December may not.
Insider Tip No. 2: Redesign Your Chart Review Workflows for Linkage
Linking chart review findings to encounter data is not simply a technical toggle - it requires a deliberate redesign of the workflow through which chart review diagnoses move from the clinical record to the risk adjustment submission. A linked submission workflow requires:
Encounter data completeness - the encounter record underlying the clinical visit must be submitted, accepted, and available in the Risk Adjustment Suite of Systems (RASS), which encompasses RAPS, EDRA, and RAS, before a chart review finding can be meaningfully linked to it
Claim-to-chart alignment - the chart review process must be structured to identify and reference the specific encounter or claim that supports the documented diagnosis, not simply the diagnosis in isolation
Vendor capability assessment - many chart retrieval and coding vendors are equipped to produce linked submissions, but not all. Plans should evaluate whether their current vendors can operationalize linkage at scale and on the required timeline
Data validation - a robust pre-submission validation process must confirm that linkage identifiers are present, accurate, and reconcilable against encounter data before submission to CMS
This is not a trivial operational lift, particularly for plans with high supplemental data volume. It requires coordination across clinical operations, data analytics, vendor management, and compliance. Plans that treat it as a checklist item will underestimate the work involved.
Insider Tip No. 3: Prioritize Your Highest-Risk HCC Codes for Linkage First
Not all unlinked diagnoses carry equal financial weight. A practical remediation strategy starts with triage - identifying the unlinked CRRs that are generating the highest risk score contribution and prioritizing those for linkage remediation first.
This means:
Ranking unlinked diagnoses by HCC coefficient - higher-weight HCCs drive larger payment impacts per member, and represent the greatest immediate revenue risk if excluded
Identifying chronic conditions most commonly captured through chart review - conditions such as diabetic complications, heart failure, chronic kidney disease, and COPD are among the most frequently documented in supplemental submissions and tend to carry significant HCC weight
Reviewing encounter data completeness for those conditions - if the encounter supporting a high-weight diagnosis was not submitted or was rejected by CMS, the linkage problem starts upstream and must be addressed at the encounter level before chart review linkage is possible
A prioritized, HCC-weighted remediation approach ensures that the highest-value risk adjustment revenue is protected first, even if full workflow redesign takes additional time.
Insider Tip No. 4: Validate Your Encounter Data Infrastructure in Parallel
The claims linkage requirement is only as strong as the encounter data it links to. Plans with encounter data submission gaps -records that were never submitted, were submitted and rejected, or were accepted but are incomplete -will find that even a well-designed chart review linkage workflow cannot produce valid linked submissions if the underlying encounter infrastructure is broken.
This makes encounter data validation an essential parallel workstream:
Encounter acceptance rates - what percentage of submitted encounter records are being accepted by CMS? Rejection rates above industry benchmarks signal systemic issues that will undermine linkage.
Encounter completeness - are all eligible clinical visits being captured and submitted as encounter records, including office visits, specialist encounters, and hospital-based services?
Diagnosis coding on encounters - even where encounters exist, are the diagnoses on those encounters coded with the specificity required to support HCC assignment? Vague or unspecified codes on encounter data weaken the linkage foundation.
Reconciliation cadence - is there a regular reconciliation process confirming that submitted encounter records are accepted and reflected in the risk adjustment data CMS holds?
Encounter data quality and chart review linkage are two sides of the same coin. Plans that address both simultaneously will be positioned to maximize risk score capture under the new rules. Plans that address only one will find themselves limited by the other.
How Alcyon and Ascend Can Help
The claims linkage requirement represents a significant operational and compliance challenge - one that most MA plans cannot absorb with existing internal resources alone. It demands clinical depth, data infrastructure expertise, vendor coordination capability, and a strategic understanding of how all these elements interact to produce accurate, defensible risk scores.

The Bottom Line
The April 2026 CMS Rate Announcement was not a proposal. It was a finalization. Unlinked chart review records will not count toward CY 2027 risk scores - full stop. Plans that have relied on standalone supplemental submissions to capture risk adjustment revenue must redesign their workflows before that revenue disappears.
The window to act is open. It will not stay open indefinitely. The plans that use 2026 to build linked submission infrastructure will enter 2027 with their risk adjustment revenue protected. The plans that wait will spend 2027 trying to recover what they could have preserved.
